Overview
A health related social needs screening is a way for organizations like ours to understand your needs and connect you to a resource who can help.
A health related social needs (HRSN) screening is different from other health screenings because the questions aren't about health problems. These are things that impact your health and well-being like lack of healthy food, an unsafe place to sleep, or inability to get a ride to places you need to go like appointments.

Everyone gets screened for health related social needs because everyone's health is impacted by these things. However, sometimes talking about HRSN's is difficult. Knowing that, Brunch N Bible Study, Inc. offers tea and a small brunch to every woman who comes in for a screening.
Every woman deserves the chance to live a holistically healthy life and Brunch N Bible Study, Inc. is committed to making that happen.
How it works
STEP 1 - GET SCREENED
Before you can get services, we need to better understand what your needs are and if you are eligible for services under this Program. To do so, we have to ask you a few questions about housing and utilities, food security, transportation, employment, education, and interpersonal safety. Screenings are only done with your consent and the information you give is only used to determine your needs and eligibility.
STEP 2 - FIND OUT WHAT YOU'RE ELIGIBLE FOR ​
If your screening identifies an unmet need and you give your consent, a Social Care Navigator will do an eligibility assessment to find out if you can receive services under this Program. They will reach out within 5 business days.
STEP 3 - GET SUPPORT ​
If you have unmet needs, want services, and are eligible, our Social Care Navigator will connect you with a provider who can help.
Navigation includes: housing, nutrition, transportation, employment, education.

STEP 4 - SERVICES BEGIN

STEP 5 - FOLLOW UP

Our Social Care Navigator will follow up to see if your needs are being met, if there are any changes to your situation, and any other supports needed.
