A health related social needs screening is a way for organizations like ours to understand your needs and connect you to a resource who can help.
A health related social needs (HRSN) screening is different from other health screenings because the questions aren't about health problems. These are things that impact your health and well-being like lack of healthy food, an unsafe place to sleep, or inability to get a ride to places you need to go like appointments.

Everyone gets screened for health related social needs because everyone's health is impacted by these things. However, sometimes talking about HRSN's is difficult. Knowing that, Brunch N Bible Study, Inc. offers tea and a small brunch to every woman who comes in for a screening.

STEP 2 - FIND OUT WHAT YOU'RE ELIGIBLE FOR ​
If your screening identifies an unmet need and you give your consent, a Social Care Navigator will do an eligibility assessment to find out if you can receive services under this Program. They will reach out within 5 business days.
